The Community Wellness Club is an initiative developed under the umbrella of The Community Hospital of Seventh-day Adventists to support proactive, accessible, and preventive healthcare services through the promotion of wellness and healthy living. It was established to foster a culture of wellness among club members with the support of medical services available at the Community Hospital.

How Enrolment Works

    Start by completing the Community Wellness Club application form online. This allows you to choose the most suitable membership tier for you.

    Complete Your Enrolment

    You will receive a secondary enrolment form based on the membership tier you chose. Fill out the enrolment form online. You'll receive an automatic confirmation email with a copy of your enrolment details and short list of documents needed.

    Schedule Your Wellness & Medical Review

    Our Wellness Team will contact you to explain the medical requirements and schedule a single, convenient visit at the hospital.

    Attend Your Wellness & Medical Review

    Your visit is completed in one coordinated process and helps ensure the plan is right for you. It includes:
    - Vital signs and basic health measurements
    - Required screening tests
    - A virtual consultation with a doctor

    Review & Approval

    Your application is carefully reviewed. Once approved, you'll receive a confirmation of acceptance and clear payment instructions.

    Make Your First Payment

    Submit your payment and send proof to our team. This activates the final stage of enrolment.

    Confirmation & Benefit Start

    Once payment is confirmed, you would then be an active member of the Community Wellness Club, with access to the benefits of your selected tier.

Who can join the Wellness Club?

Anyone from 18+, Individuals, families, and even employees of organizations can sign up for Bronze, Silver, or Gold. Gold members, with the exception of dependents, must be aged 18–59 years of age.

How to sign up for Gold?
  • Fill out the enrolment form online with necessary documents
  • Complete an insurance medical (at your own expense) and submit signed and completed forms
  • Await notification of acceptance and terms of coverage from the insurance administrator
  • Make payments and start reaping benefits after 3 months of enrolment
What happens if I pay late?

Late payments may lead to suspension or termination of benefits (see our missed payments policy).

Do I have to be a patient of the hospital to join?

No, you can join as a community member. However, the biggest savings and benefits are applied when you use the hospital’s services.

Can I switch tiers later?

Yes! You can upgrade at any time to enjoy more benefits, including Gold’s group health plan.

Why should I use the hospital instead of going elsewhere?

Members get exclusive discounts only at our hospital — meaning your membership goes further when you keep your care in-house.

When does my group health coverage start?

Coverage begins once your Gold Tier enrollment is approved and your first payment is received.

When can I start making claims?

Claims can be made after a period of three months from the date of acceptance.

How do I make a claim?

Pay for the service, submit your receipt with the filled and stamped claim form, and reimbursement will be processed based on your plan benefits.

Is there a waiting period before I could use my benefits?

Yes. After a period of 3 months from the date of acceptance, you can start receiving the benefits of the plan.

How long does it take to be reimbursed?

Claims are usually processed within 10–14 business days once all required documents are submitted.

What makes the GOLD tier special?

Gold includes added benefits like comprehensive health coverage, giving you more protection and value for your money.

Can I use my membership at other hospitals or clinics?

Membership benefits are exclusive to our hospital and affiliated services.

Do I get priority access as a member?

Yes! Members enjoy faster service and priority scheduling for certain services.

What kind of services do I get discounts on?

Depending on your tier, you’ll enjoy savings on doctor visits, lab tests, imaging, wellness services, and much more.

Can I join online or do I need to come to the hospital?

Our enrolment form is digital and can be accessed online or sent via link from our Wellness Coordinator.

Does the membership cover emergencies?

With the Gold tier, emergencies are covered externally up to a value of $12,000. Terms and conditions do apply.

How does being a member save me money?

Members enjoy discounts instantly, especially Gold members. Services done at the hospital are covered upfront—so you only pay what you owe, with no need for tedious claims or waiting periods.

Can I have this if I already have private insurance?

Yes. If you already have insurance, you’ll pay the bill upfront and claim from your primary insurer first. Once they reimburse you, CHHP will cover any eligible costs your primary plan didn’t pay.

What happens if I miss a payment?

1. Missed Payment: Benefits may be temporarily suspended
2. Missed Payments: Suspension + warning
3. Missed Payments: Coverage terminated (re-enrollment + medical exam required)
